THIS FACILITY WAS CLOSED BY THE VILLAGE OF ST. ANNE IN JANUARY.
-THE FRONT DOOR HAS BEEN REPAIRED -THE HVAC HAS BEEN REPAIRED -THE FACILITY HAS CLEANED AND REMOVED PEST ISSUES. THE VILLAGE LIFTED THE CLOSURE ABOUT 3 WEEKS AGO AND THE FACILITY HAS BEEN OPERATING SINCE. DURING THIS INSPECTION, I OBSERVED THAT ALL OF THESE ISSUES HAVE BEEN CORRECTED. A RISK ASSESSMENT WAS CONDUCTED DURING THIS INSPECTION. |
